and these Terms will remain in effect while you use the Services. These Terms include the provisions in
this document as well as those in the Privacy Policy.
Typically our contact us forms will request information such as your name, phone number, e-mail address, and what portion of the day is best for us to call you, along with other information you choose to provide to us in any spaces we provide into which you can type a message, but please do not include sensitive information about yourself or others, including without limitation personal health information, financial information, social security numbers, or other similar information. In all cases the personal information we collect through our Site is that which you provide in the various forms.

SMS/Text Messaging Program
By providing your consent, you agree to receive text (SMS/MMS) messages from Constella Health as described in this section.
Constella Health will not share or sell your mobile phone number or text messaging consent to third parties for marketing purposes. Your information is handled in accordance with our Privacy Policy and applicable law, including HIPAA. Carriers are not liable for delayed or undelivered messages.
Constella Health sends text messages to patients to coordinate care, follow up on program enrollment, and provide appointment reminders and confirmations. Messages are sent by your care team and may include automated appointment reminders. This is not a marketing program.
You consent to receive these messages during a phone call with a member of your care team or through a written or electronic consent form. Consent is not a condition of receiving care.
Message frequency varies based on your care needs and interactions with your care team. Message and data rates may apply, depending on your mobile carrier and plan.
You can opt out at any time by replying STOP to any message. After you reply STOP, we will send a confirmation and will not send further texts unless you opt back in.
